The 4-way 2-pole is indeed not enough for what you want to do. The above diagram from BL is really ingenious - it utilizes the standard Strat switch to an enormous extent.

Anyways, I'm not aware of a 4-way superswitch, at least not in blade form. There are many rotary switches that would fit the bill, but I guess they're out of question here.

The phase inverting with the 1/2 out of phase cap is very easy, just look at the diagram below. The switch is an SPDT on/on type, like the usual push-pull and push-push pots.
